האם ספורט מועיל לתפקוד רצפת האגן?

תקציר 

פעילות גופנית הנדרשת בספורט ההישגי מעלה את הסיכון לדליפת שתן. השכיחות היא 25% כאשר אצל כל ספורטאית חמישית הדליפה תהיה על רקע מאמץ. עצימות גבוהה מעלה את השכיחות, לדוגמה, בענף הכדור עף השכיחות היא 75%. הפרעות נוספות קיימות הן במערכת העיכול והן בתפקוד המיני. עיסוק בספורט הוא גורם סיכון. ההנחה שהגורם לדליפת השתן הוא חולשה של שרירי רצפת האגן אינה עולה בקנה אחד עם מדדי הכוח שנמצאו טובים יותר בקרב ספורטאיות. כנראה שהגורם העיקרי הוא עליית הלחץ התוך-בטני המופעל כלפי מטה אל פתחי רצפת האגן, לחץ העולה על יכולת התמיכה של רקמות רכות ושל שרירי רצפת האגן  בעת המאמץ. מהו הגורם המשפיע בספורט ההישגי על תפקוד רצפת האגן? שאלה זו נחקרה מעט במדידה אובייקטיבית של תפקוד רצפת האגן תוך כדי ביצוע המאמץ הגופני. כך שמנגנון הפגיעה ברצפת האגן עדיין אינו ברור. מטרת סקירה זו היא לבחון את הידוע בספרות על פעולתם של שרירי רצפת האגן בעת המאמץ הגופני, ולנסות להבין את הקשר לשכיחות הגבוהה של הפרעות תפקודיות אצל ספורטאים. בוצע חיפוש במאגרי המידע Scholar Google ,PubMed ו-Database Library Cochrane עם קריטריונים להכללה: Elite Athletes+Pelvic Floor Muscle Function, בעשור האחרון, ונמצא מחקר אחד בלבד שהציג נתונים באמצעות כלי מדידה אובייקטיביים בעת הפעילות גופנית. מיעוט הידע וההבנה של מנגנון הפגיעה והעדר פרוטוקול טיפולי מתאים בספורטאים מעלים את הצורך במחקרים קליניים שיבחנו את השפעת רכישת מיומנות השליטה בשרירי רצפת האגן על הפחתת התופעות המביכות בפרט ועל הישגים בכלל. בינתיים, חשיבות רבה טמונה בהערכה מקצועית של פיזיותרפיסטית מיומנת לכל ספורטאי/ית שסובל/ת מהתופעה. על פי הקווים המנחים בעולם, הטיפול הראשוני והיעיל ביותר הוא שמרני, טיפול  פיזיותרפי לשיקום רצפת האגן, הכולל הכוונה התנהגותית ולימוד שליטה בשרירי רצפת האגן, בעיקר בעת מאמץ.

Is sport beneficial for pelvic floor function?

Abstract

The degree of physical activity that is required in competitive sports increases the risk of urinary incontinence (which occurs in every one-in-five female athletes during physical exertion), as well as disorders relating to the digestive system and to sexual function. The assumption that urinary incontinence stems from weakness of the pelvic floor muscles (PFM), however, is not in line with strength indicators, that have been found to be increased among female athletes. As such, the main cause seems to stem from an increase in intra-abdominal pressure that is applied downwards towards the openings of the pelvic floor, that exceeds the support capacity of soft tissue and of the PFM. When asking which factors impact the pelvic floor function in competitive sports, some studies show objective measurements of PFM while performing physical effort. The aim of this paper, therefore, was to review the literature regarding PFM activity during physical exercise. First, a search of the following keywords was performed: Elite Athletes + PFM Function; in the last decade; Yet only one study was found that presented objective data. The lack of understanding of the related injury mechanism, and the lack of therapeutic protocols in high-exertion sports, increases the need for clinical studies that will examine the effect of acquiring skills for controlling PFM. Until such time when the literature is scarce in objective studies on the topic, it is vital that a skilled physical therapist be available for female athletes who suffer from pelvic floor dysfunction. According to existing guidelines, the initial and most effective treatment is conservative physical therapy for pelvic floor rehabilitation, which includes behavioral guidance and learning to control the PFM, especially while exerting physical effort.
